When Do You Need a Medication Review?

A medication review is recommended if you:

  • Take several medications on 3 or more long-term prescription medications
  • Live with chronic conditions such as diabetes, hypertension, heart disease, or a mental health disease

  • Recent hospital admittance Have recently been discharged from hospital

How Medication Reviews Work at Laurel Pharmacy Clinic

  1. We Let You Know – our pharmacist will always indicate when they are conducting a formal review.
  2. Information Gathering – we may ask if you’ve done reviews at another pharmacy and request your consent to obtain a copy for continuity of care.
  3. Comprehensive Review – during the consultation, our pharmacist will assess all your prescription, OTC, and supplement use.
  4. Collaboration – we work with your doctor or other healthcare providers if adjustments are needed.
  5. Written Summary – you will receive a clear, easy-to-understand record of your medication plan.
  6. Follow-Up – we provide ongoing support and schedule follow-ups as needed.

Medication reviews can be completed in person or virtually, depending on your preference.

At Laurel Pharmacy Clinic & Compounding, our pharmacists provide two main types of Alberta Health–funded medication reviews:

  • SMMA (Standard Medication Management Assessment)
    This is a one-time comprehensive review of all your medications. It is designed for patients who are taking multiple prescriptions, have chronic health conditions, or are at risk of medication-related problems. The goal is to make sure your treatment is safe, effective, and easy to follow.
  • CACP (Comprehensive Annual Care Plan)
    This service goes a step further. It is available to patients who live with certain chronic conditions such as diabetes, asthma, COPD, hypertension, or heart disease. A CACP includes a detailed long-term care plan, developed by your pharmacist in collaboration with your doctor, to help you manage your health year-round.

Both services are covered by Alberta Health (if you meet eligibility criteria), and our pharmacists will let you know which service applies to your situation.

Yes. Please bring all your prescription bottles, OTC products, and supplements to your appointment.

Yes. As per the Alberta College of Pharmacy standards, we are required to share a copy with your physician or specialist to ensure coordinated care.

Alberta Health only covers one initial medication review per year. If you have already completed a review at another pharmacy, you may not be eligible for another covered review until the next year.

If our pharmacist indicates that a review has already been completed elsewhere and you are not aware of it, you can:

  • Contact the pharmacy where the review was recorded to request clarification, or
  • Contact the Alberta College of Pharmacy if you believe a review was done without your consent or knowledge.

This ensures that your rights as a patient are protected and that medication reviews remain accurate and transparent.

Yes. We offer virtual consultations for patients who prefer the convenience of reviewing medications from home.
